Demonstrating Value in Denver
|
|
The OCS
Summit is moving to Denver, Colorado on
October 6, 2007,
just prior to the NAHC
Annual Meeting. Join us for this one day
learning intensive, focused on demonstrating value in
home care.
You will have the opportunity to learn and share with
the most progressive leaders in home care and
hospice who have demonstrated value in areas such
as payment reform, patient satisfaction, managed
care, etc.
We will honor those agencies that have demonstrated
value through our Vision
Awards. Award winners will be announced
during the OCS Summit. Vision Awards honor home
care, hospice and private duty organizations for
strategic and/or innovative use of OCS products in
furthering agency performance. These awards
acknowledge creative problem solving efforts that lead
to improved quality of care or agency operations.

Martha Tecca, Senior
Director of Hospice and Peer Forum will be
on the road, participating in the Iowa Hospice Organization's Regional
Workshops on Data-Driven Management ,
June 26 - 28,
2007. Martha will be leading workshops on
the following dates and locations, providing new
educational material and facilitating group
sharing.

General Program
Grant funds work to
- Improve availability and quality of community-
based and institutional long-term care programs
- Expand opportunities for older persons to play
meaningful roles in society
- Support applied and policy research into causes
and solutions of significant problems of the aged
- Increase number of professionals and
paraprofessionals adequately prepared to serve the
elderly
Proposal must fall into one of four categories
- Model or service projects
- Advocacy for system change
- Education and training
- Research
Proposals must include plans for disseminating
findings or project outcomes.
Rotating Submission Deadlines - 2/1, 5/1,
8/1
Funds awarded within 4 to 6 months of deadlines;
average grant is $64,000.00.
Multi-year awards allowed.
The
Foundation has a strong interest in serving
the Chicago metropolitan area. Where projects of
equal significance are under consideration, priority is
given to proposals that benefit this geographic area.
Applicant must have tax exempt status under Section
501 (c)(3) of the Internal Revenue Code.
Consider partnering with OCS for assistance with
research proposal, data, analysis, interpretation, or
dissemination.
