Hospice Leaders Project

OCS announces
that HomeCare
HomeBase, located in Dallas, Texas, has
joined The Hospice
Leaders Project.

This voluntary program expedites the adoption of
meaningful performance comparisons in the hospice
market. Through this program, vendors will provide
hospices with standard definitions, systems and
guidance to capture and report valuable patient
management data, facilitating performance
improvement and participation in state, national, and
private quality initiatives. The
Hospice Leaders Project is being
coordinated by OCS.
The Hospice Leaders
Project is a key part of the multiple efforts
to formalize quality measurement, leveraging the vast
experience of thousands of hospices and participating
organizations. The proposed Medicare QAPI Conditions of
Participation require measurement, but do
not specify indicators. Several national and state-
sponsored organizations are promoting frameworks
for quality and data-centric improvement, including the
National Hospice and
Palliative Care Organization (NHPCO), National
Quality Forum (NQF), American Hospice
Foundation, the National Association of Homecare
and Hospice (NAHC) and the CMS-funded
Carolinas Center for
Medical Excellence.
The Hospice Leaders
Project will expedite standardized data
information, collection and reporting for hospice
agencies enhancing the impact of all of these diverse
programs.
