PatientView

OCS introduces PatientView, a new patient-level decision support service which predicts the risk a patient has of experiencing acute care hospitalizations or emergent care during service.

This real-time service takes the unique characteristics of each patient at the start of care, identifies their risk of needing hospitalization or emergent care, prioritizes those patients into risk levels, and automatically returns the information to the provider in real time.

Providers use this information to positively affect patient outcomes and  reduce their exposure to these costly events.

PatientView includes patient-level analysis and risk stratification reporting on the probabilities of acute care hospitalization and emergent care. In addition to the risk level, the patient-level reporting function provides patient identification (patient ID (M0020), name (M0040), SOC date (M0030), and date of birth (M0066)) and diagnosis information (primary (M0230), first secondary (M0240-1), and payment (M0245).
